DPP-4 Inhibitors
Sitagliptin
通用名:Sitagliptin | 常用商品名:Januvia
What is Sitagliptin?
Sitagliptin is a DPP-4 inhibitor that enhances the body's own incretin hormones to improve blood sugar control with low risk of hypoglycemia.
How it works
It blocks the DPP-4 enzyme that breaks down GLP-1 and GIP, leading to more insulin release after meals and less glucagon when glucose is high.
Common uses
- Add-on therapy to metformin or other oral agents in type 2 diabetes
- Option for patients needing glucose control without weight gain
- Useful when hypoglycemia risk must be minimized
Monitoring & Safety
- Labs: Track hba1c, glucose, and egfr to adjust dosing in kidney impairment.
- Side effects are usually mild; rare pancreatitis warrants prompt evaluation.
Dosing (简要)
- Once daily dosing; standard 100 mg, with reductions at lower eGFR levels.
- Can be taken with or without food.
Drug interactions
- Few major interactions; watch combined use with sulfonylureas or insulin for hypoglycemia.
- Certain diabetes drugs may need dose adjustments when added.
Patient tips
- Continue diet and exercise efforts; medication works best with lifestyle changes.
- If you develop severe abdominal pain, stop and seek care for possible pancreatitis.
- Keep a glucose log to review at follow-ups.
When to contact your doctor
- Severe or persistent stomach pain, especially radiating to the back.
- Unusual rash or joint pain.
- Repeated low or high glucose readings despite adherence.
