Triple-phase bone scan demonstrates hyperemia and focal delayed uptake around the femoral stem of the right hip arthroplasty, most prominent proximally.

Why it matters

Increased periprosthetic uptake with this pattern favors mechanical loosening. Diffuse intense uptake in all three phases would raise more concern for infection.

• Correlate with ESR/CRP; if elevated or symptoms concerning, consider labeled WBC scan or aspiration to exclude infection.
• Orthopedic consultation for possible revision if pain and functional limitation persist.
