What does the scan show?

Markedly increased tracer uptake throughout the right innominate bone and proximal femur, respecting joint spaces and showing the "blade of grass" configuration typical of Paget changes.

Why it matters

Paget disease can lead to pain, deformity, and secondary osteoarthritis. High-turnover lesions also raise fracture risk and rarely transform to osteosarcoma, so accurate mapping is important.

Recommended next steps

• Correlate with alkaline phosphatase level to gauge activity.
• Consider bisphosphonate therapy if symptomatic, high-turnover labs, or weight-bearing bones are involved.
• Repeat bone scan or targeted SPECT/CT after treatment to document response.
